Payment
- Payment due at time of registration
- Credit card payments are accepted online only via the registrant's own device
- Checks are accepted on-site only if on-site registration is permitted for that course
Cancellation
All registrations are subject to this ULSD Cancellation Policy: We reserve the right to cancel, modify date, revise course content, revise program faculty and activities. Pre-registered participants will be notified if an activity is revised or cancelled, as warranted. In the event an activity is cancelled, tuition will be refunded.
Non-limited Attendance Activity Cancellation Request
ULSD must receive your written request for cancellation not later than 2 weeks prior to activity start date to process a refund for a "non-limited" attendance activity. A 30 percent processing fee will be deducted from all eligible refunds. A "No-Show" will forfeit activity tuition.
Limited Attendance Activity Cancellation Request
ULSD must receive your written request for cancellation not later than 4 weeks prior to activity start date to process a refund for a "limited" attendance activity. A 30 percent processing fee will be deducted from all eligible refunds. A "No-Show" will forfeit activity tuition.
Academic Freedom, Professional Differences, Use of Limited Knowledge and Sponsored Activities
Educational institutions have the responsibility to disseminate new knowledge. In so doing, some activities may include controversial procedures, materials or commercial references. Please be advised that there are potential risks of using limited knowledge when incorporating new techniques and procedures into the practice of dentistry without supervised clinical experience and/or competency tests in the techniques or procedures being taught. These activities do not necessarily reflect the philosophies of the University of Louisville pre-doctoral or post-doctoral training programs. Any activity sponsors will be identified in promotional material and all relevant financial relationships will be disclosed and mitigated, per ADA CERP standards.

Certificates, Attendance Verifications and Record Keeping
Each attendee is responsible for maintaining their own records and for reporting their own CDE activities to all appropriate bodies in accord with any jurisdictional and/or membership requirements. The University of Louisville School of Dentistry Office of Continuing Education will maintain information for no more than 6 years from date of course. Certificates and attendance verifications dating back to 2012 are available in your personal profile.
Professional Decorum
The University of Louisville Office of Continuing Education program maintains a Code of Professional Responsibility which defines certain expectations for all participants.

- Integrity
- Self-responsibility
- Promoting professional standards
- Acing legally
- Respect for others' property
Failing to meet these professional expectations will result in an automatic failure and forfeiture of full tuition.
Reduced Tuition for Faculty, Residents, Staff and Students
ULSD Faculty, Residents, Staff and Students may qualify for reduced tuition. Please contact the Office of Continuing Education for more information.
